What are Zygomatic Implants and who is suited for this procedure?

Zygomatic implants, also known as zygomatic dental implants, are specialized long implants anchored in the zygomatic bone (cheekbone) rather than the maxilla. This procedure is particularly suited for patients with significant bone loss in the upper jaw who are not candidates for conventional implants. While traditional dental implants typically measure up to 18 mm in length, zygomatic implants can be as long as 55 mm.

They are ideal for patients with severe bone loss and do not – unlike traditional implants - require bone grafting. This is because the zygomatic bone experiences minimal bone loss after tooth extraction, providing a stable foundation for the implants even in patients with significant jaw atrophy.

Zygomatic teeth implants provide a reliable solution for severe bone loss, offering an alternative to extensive bone grafting procedures. They are commonly used together with the All-on-4 treatment concept, allowing for the immediate placement of a fixed prosthesis. This treatment can restore a full set of teeth in a single day, even for patients with minimal bone in the posterior maxilla.

Zygomatic dental implants are suitable for:

  • Patients with significant bone loss in the upper jaw

  • Long-term denture wearers

  • Those avoiding bone grafting procedures like iliac crest bone, etc.

  • Patients requiring a quick, one-day procedure for fixed teeth Individuals in good health, without acute sinusitis

Treatment Process for Zygomatic Implants

Diagnosis

The process begins with three-dimensional imaging (either CBCT or CT) to analyze the jaw structure. This step is crucial for understanding the bone anatomy and planning the procedure.

Digital Planning

The implant positions are digitally mapped out. This ensures precise placement during surgery.

Measuring Facial Proportions

Measurements of facial proportions are taken to design the long-term prosthesis. This involves collaboration with dental technicians to ensure an aesthetically pleasing outcome.

Surgery Simulation

For complex cases like Quad-Zygoma treatments, a surgery simulation may be conducted using a 3D-printed skull model. This helps the surgical team prepare for the procedure.

Pre-Operative Consultation

Aconsultation with the anesthesiologist is conducted to assess the patient's health, including blood tests and ECG, to ensure they are fit for general anesthesia.

Surgical Procedure: Zygoma implant surgery

The zygomatic implants are placed under general anesthesia. After the surgery, an initial impression for the dental prosthesis is taken.

Recovery Phase

Post-surgery, the patient enters a recovery phase where the provisional prosthesis is crafted by the in-house dental laboratory.

Fitting of Healing Denture

The first fitting of the provisional prosthesis occurs, with adjustments made as necessary.

Placement of Provisional Prosthesis

The provisional prosthesis is secured, and the patient is discharged with instructions for care.

Follow-Up and Maintenance

Follow-up appointments include professional cleaning and maintenance, along with instructions for at-home care.

Final Prosthesis

After approximately 4-6 months of healing, the final prosthesis is fabricated and placed, providing the patient with a permanent solution.

Frequently asked questions regarding zygomatic implants

How much do zygomatic dental implants cost?
A treatment with zygomatic implants is approximately 40-60% more expensive in the upper jaw compared to standard All-on-4 treatments. However, it remains more cost-effective than time-consuming procedures involving iliac crest bone grafts and traditional implants.
Is the prosthetic process different with zygomatic implants?
The prosthetic process with zygomatic implants is similar to the All-on-4 treatment. The difference lies in the bone anchorage, which is not visible or felt by the patient.
Do zygomatic implants feel different from regular implants?
Zygomatic implants, like traditional implants, end in the upper jaw and are connected to the prosthesis. They do not feel different from regular implants.
How many zygomatic implants can be placed?
Typically, the zygomatic bone can accommodate two implants per side, allowing for a secure attachment of a full upper denture on four zygomatic implants in cases of extreme bone loss.
What is the relationship between zygomatic bone implants and the All-on-4 concept?
Zygomatic implants are essential for All-on-4 treatments in cases of severe bone loss in the upper jaw's posterior region. They provide the necessary stability for immediate loading of a full arch prosthesis.
How long do zygomatic implants last?
With proper care, zygomatic implants can last a lifetime. They have a documented success rate of nearly 100% after five years and around 92% after ten years.
